What should I know about my first physical therapy appointment?



 


  • Upon your arrival at our office, you will be registered into our electronic system. We will help you understand your insurance benefits and treatment process so that you can focus on having the best recovery possible.



 


  • Next, our physical therapist will meet you for your initial assessment. They will ask questions about your medical history, current pain, or injury during your assessment. The more details and events you can remember that you may think relate to your condition the better!



 


  • Your movement, balance, muscle endurance, strength, coordination, and much more will be assessed during your appointment. Our goal is to help you, so please make sure that you communicate with our therapist if anything hurts or bothers you during the examination!



 


  • Expect to receive treatment on your first day and walk out feeling better than when you entered!



 


  • Our physical therapist will recommend and prepare a customized course of treatment to ensure the quickest recovery and the best results. You will be taught one or two simple movements to perform at home that will be considered your ‘home exercise program’ (HEP). This will help further facilitate improvement of your movement & symptoms.



 


  • After your therapist has finished, you will schedule a series of visits at the front desk. The amount of therapy needed will be dependent upon the advice of your therapist, and will also be discussed with you at your first visit.

We are out of network with most insurance plans at Central Park Physical Therapy. If you have out of network benefits you may be fully covered, and we will submit claims for you.


 

We understand that insurance can be perplexing, and we will check your health benefits before you arrive, as a courtesy to determine whether there will be a copay, deductible, or coinsurance for treatment services.


 

We do accept new Medicare patients.

Will I notice any discomfort during my treatments?

Good communication with our therapist will help minimize any discomfort. We will teach you how to be on your way to quick pain relief and back to normal activities.


 

Physical therapy is not meant to hurt, but to relieve pain! There is a chance, however, as you restore your mobility that your pain levels will fluctuate. You might experience some temporary soreness from using muscles that are weakened or recovering.

What can I expect from physical therapy treatments?


 

With physical therapy, it’s important to know that the overall goal is to restore pain-free mobility and function and to prevent the injury or pain from recurring.

Physical therapists are trained medical experts when it comes to musculoskeletal and neurological conditions. While every patient’s condition and pain are unique, our physical therapist will use a variety of gentle hands-on techniques, exercises, and equipment to relieve pain quickly and restore your function to optimum levels. 

How long will each of my sessions be?

Although all patients are different, typically, the first session lasts for 1 hour 15 minutes. Each session after that may be 50-120 minutes.


 

The amount of time will be based on your specific needs, and our therapist will discuss this with you. You will most likely see your therapist once or twice per week, to begin with, and as you progress through treatment, your sessions will be more spread out.

How should I dress for my appointments?

Dress comfortably in clothes that allow easy access for the therapist to examine your problem areas. For example bring shorts and T-shirt, halter or sports bra.


 

For treatments, loose-fitting clothes that are comfortable for gentle exercises and that don’t restrict movement, be prepared to remove shoes/footwear.

When can I expect to notice relief?

Immediately! You can expect to benefit from every single session, and you will even be taught how to 'fix yourself' the days you are not visiting us.


 

Your plan of care and number of visits will be determined during your first visit and explained to you by our physical therapist.

How do payments work?

Your insurance benefits will be determined before scheduling, and you will be informed of what, if any, payment is necessary prior to that date.


 

We accept cash, check, debit card, and credit card. We are happy to discuss payment plan options as well. It is our policy that payment is made at the time of service. 

Can I continue with physical therapy even when my treatment plan ends?

Yes, once you have completed your therapy sessions, our therapist will review your at-home continuing therapy program.


 

We can also make arrangements to meet you at your gym to work with you on determining what equipment may be beneficial, and what exercises would be helpful in your continued recovery.

What should I do to prepare for my first appointment?

Please have your prescription (not needed) and insurance card available when you call. This will allow us to verify your insurance benefits and provide you with an estimate of coverage.


 

To bring on your first visit:



 


  • Print out the patient forms, fill them out, and bring them with you. If you are unable to do this, please allow for extra time to fill these out upon your arrival.


  • Please wear comfortable clothing that allows easy access to your problem areas.


  • Please arrive 15 minutes early to complete any remaining paperwork, so you can maximize your time in your evaluation.


  • Be prepared to talk about how your condition has affected you, for how long, and when you notice symptoms changing.


  • Insurance information (we will make a copy of your card).


  • A prescription (if needed) for physical therapy from your referring physician, ARNP, or PA.


 

In most cases, insurance will cover a good portion, if not all, of your treatment costs. We understand navigating insurance can be confusing, so Central Park Physical Therapy will be happy to assist in contacting your insurance company and determining your coverage based on your policy.

Back pain vs. sciatica

Back pain

“Back pain” is a broad term that can be caused by an array of different conditions. For example, you may experience back pain due to poor sleeping position, improper sitting posture, a fall, or an injury sustained while lifting something with poor mechanics. 


 

Back pain can be described as acute, meaning it is short-term, or chronic, meaning it is long-term (typically lasting for three months or longer).


 

The treatment plan that our New York physical therapist sets up for you, will depend on how you developed the back pain, in addition to its exact location and your past medical history.  

Sciatica


 

Sciatica is a type of back pain that is caused by irritation to the sciatic nerve. It is considered a type of lumbar radiculopathy. Sciatic nerve pain originates from one or more spinal nerve roots in the lumbar (lower) and/or sacral spine.

Any type of pain and/or neurological symptom that originates from the sciatic nerve is referred to as sciatica. The symptoms of sciatica are typically felt along the path of this nerve. The sciatic nerve begins at your lumbar and sacral spine and then extends further down to your buttocks, legs, and finally ends at the bottom of each foot, however as the nerve branches out it changes its name. 

The sciatic nerve can become compressed or irritated, which causes a “shooting,” “stinging,” or “burning” sensation in your lower back, buttocks, legs, or feet.

What’s causing my back pain/sciatica?

Most commonly with sciatica, people lose flexibility in the lumbar spine, hips, and pelvis, causing the surrounding muscles to tighten. As a result, the spine mechanics become altered, and the sciatic nerve compresses as it travels through these tissues.

Sciatica can be caused by a variety of conditions, including arthritis, mechanical compression, segmental instability, repetitive strain, or any injury directly to the sciatic nerve. It can also be indirectly caused by an injury to another part of your body that creates strain somewhere else, for example: imagine hurting your big toe so bad that it causes you to limp and lean over from your waist eventually causing your lower back muscles to strain. Even if your toe recovers on its own, your lower back may not.

Herniated discs and other underlying conditions can also cause severe back pain and radiculopathy in your thigh, leg, or foot.

Degenerative disc disease is a common condition that causes back pain as we age. Those living with this condition typically experience dull, aching pain in their lower back, in addition to difficulty standing or walking for extended periods.

There are numerous reasons why you might be experiencing this type of pain, and the best way to find out for sure is to see a physical therapist. Most of the time there are multiple reasons and areas of your body contributing to your symptoms.

What could be causing your neck pain?

Injuries to muscle tissues and bone caused by repetitive strain are the most common reason for neck pain, however, certain degenerative conditions can also lead to neck pain. 

Some common conditions that give you pain in your neck include:

Illnesses

Certain diseases and ailments can result in neck pain, such as rheumatoid cancer, arthritis, and meningitis.

Muscle strains

Sitting at your desk all day with your neck craned toward the computer screen can cause your neck muscles to become strained by overuse.

Compressed nerves

If you have a herniated disc in your neck, or a narrowing of the foramen (space where the nerves exit the spine), the nerves that branch out from your spinal cord can become compressed. In some cases, nerve pain can even extend from the neck down to the hands and fingers — a condition known as "radiculopathy."

Injuries

Whiplash is typically sustained by a high speed or high-impact collision. This is known as a “strain” injury, causing the head to whip rapidly back-and-forth, thus affecting the soft tissues in the back or front of the neck.

Common conditions resulting in shoulder pain

Some of the most common causes of shoulder pain include: 

Tendinitis

Tendinitis occurs when the shoulder joint is excessively overused typically due to the physical demands of a person’s job, overhead activity, or sport. 


 

Another factor for tendinitis is also poor posture because this alters the normal forces on the tendons, and can set you up for tendon injury. 


 

This causes the tendons to undergo ongoing inflammation, resulting in swelling and painful impingement when raising your arm. 

Impingement

Impingement is when your shoulder joint compresses one of the tendons that pass through it. It typically occurs because of abnormal movement and tracking of the humeral head due to inefficient rotator cuff muscles. 

Arthritis

The two main forms of arthritis that affect the shoulder are osteoarthritis and rheumatoid arthritis.  Both of these result in loss of motion, weakness to the shoulder muscles, and difficulty performing normal, daily tasks.


 

Rheumatoid arthritis occurs when the immune system attacks the membranes surrounding the shoulder joint, resulting in pain and inflammation.


 

Osteoarthritis occurs when the cartilage in the shoulder joint experiences significant “wear and tear” and the bone surface becomes less smooth. By releasing the tension surrounding the shoulder joint, you can prevent the pain-causing friction.

Frozen shoulder

Frozen shoulder results in a painful loss of motion in the shoulder with a tightening of the shoulder capsule that severely limits motion. It is also known as “adhesive capsulitis.”


 

Properly functioning shoulders

The shoulder is a "ball-and-socket" joint, which means that the head of the upper arm bone, or humerus, fits into the corresponding space within the shoulder blade, or scapula, and the ends of the bones are protected by a thick layer of cartilage, preventing friction between the bones.

Tendons are also protected from rubbing against the bones by fluid-filled sacs known as bursae. Tendons connect the bones of the shoulder to a group of muscles called the rotator cuff.


 

Since your shoulders are supposed to have a lot of mobility, they sometimes become unstable and cause issues.  The type of pain you experience in your shoulder may differ depending on what is causing it. Pain from impingement, for example, typically occurs when you raise your arm from a specific point in the range of motion.

The connection between hip and knee pain

When we're in pain and go to the doctor to find out what's wrong, we're often surprised to learn that the area in question isn't a cause for concern. This is because all of our body parts are interconnected and work together to keep us alive. Often, if your knee hurts, your hip will as well, and vice versa!


 

Pain in the hips and/or knees may be caused by the joints themselves, or it may be the result of an underlying condition in another part of the body.

Your hip is a ball-and-socket joint that works to support the weight of your upper body, relying on a variety of muscles and tissues to keep it mobile and stable so it can function properly.

Your knee is a hinge joint that can only move forward and backward. When you squat, your knees support six times your body weight more than your hips. Proper hip and knee movement allow you to perform complex movements such as standing, walking, running, and dancing without falling over.

The hips and knees are part of the same kinetic chain, which means they are a group of weight-bearing joints that must work in unison for the body to function properly.

As a result, issues with your knees can lead to issues with your hips, and vice versa. Stress and deterioration may be placed on another part of the kinetic chain if one part of the chain is out of balance. It's all intertwined!

Common causes of hip and knee pain

Hip and knee pain can occur for a variety of reasons.

Because the sciatic nerve runs through both areas, hip and knee pain can occur as a result of a pinched nerve. An unbalanced posture or gait can cause abnormal stress and premature wear and tear in your hips and knees, leading to painful arthritis symptoms.

Because both hips and knees are constantly used, overuse injuries such as tendinitis and chronic muscle strain are also common. Acute injuries, such as sprains, strains, and dislocations, can also have an impact on these joints.

Some painful conditions may be limited to one or both joints. Cartilage injuries, known as labral tears, are specific to hip pain, whereas bursa sac inflammation, known as bursitis, is specific to the knee joint.

However, painful injuries that cause hip instability can also affect the knees. Tight hip flexor muscles and weak gluteus medius muscles can cause the hip to rotate inward unintentionally. This can result in painful issues like iliotibial band friction syndrome or patellofemoral stress syndrome, as well as general added stress on the knee or kneecap.

Although all of these reasons are plausible, seeing a licensed physical therapist for a thorough assessment of your condition is the best way to determine the true cause of your hip or knee pain.

Fallen arches

Your foot contains several tendons that work together to form your arch. When the tendons pull together properly, your foot will form a proper arch. 

However, when the tendons do not pull together properly, your foot will form a very small arch or no arch at all. This is referred to as “fallen arch” or “flat foot” and can cause pain or discomfort with time.

Tendinitis

Tendinitis occurs from excessive overuse of the tendons, causing them to become inflamed for a long time.  

Tendinitis can pose challenges to even the simplest of activities, such as reaching to grab a glass from the top shelf of the cabinet or bending down to pick up a dropped item. To make it worse, tendinitis can worsen over time, becoming chronic and incredibly painful. 

Strains and sprains

Sprains and strains happen often, and oftentimes, they get confused with each other! A sprain occurs when a ligament is stretched too far or torn, while a strain occurs when a muscle or tendon is stretched too far. 

Both of these injuries should be checked out by a medical professional to ensure no lasting damage will occur.

Tarsal tunnel syndrome

Tarsal tunnel syndrome occurs when the tarsal tunnel (the nerve that runs from the ankle to the foot) becomes pinched and painfully inflamed.

Symptoms include sharp, shooting pains, feelings of “pins and needles” and burning sensations in the feet.

Plantar fasciitis

Plantar fasciitis happens when the connective tissue on the bottom of the feet becomes inflamed. 

This can occur from aggravated overuse, such as constant standing, improper posture, or footwear that lacks proper arch support.

Fractures

A fracture occurs from excessive force or trauma to the bone, causing it to crack or break entirely. 

A fracture to the foot or ankle could result in weeks of downtime to ensure proper healing.

Bursitis

Bursitis occurs when the tendons rub against the “bursae” or anti-friction sacs, causing them to become irritated and uncomfortable. 

This condition can also occur in a person’s elbow, shoulder, and hip, although the foot and heel are most commonly affected.

What causes foot/ankle pain to develop?

Your feet and ankles are complex mechanical structures that must work in tandem to function properly. The foot has about a dozen bones, 30 joints, and over 100 muscles, tendons, and ligaments. Pain in your feet and ankles is very common due to all of the complicated structures that make up these structures.

A variety of events can occur in your life that can cause foot and ankle pain. A foot or ankle injury can cause excruciating pain that makes it difficult to bear weight on the affected area.

Cubital Tunnel Syndrome

Cubital tunnel syndrome is a common form of nerve compression damage that can lead to elbow, wrist, and hand pain. This condition is different from carpal tunnel syndrome and occurs when pressure is put on the cubital tunnel and surrounding nerves.  

It is caused by repetitive pressure on the ulnar nerve, which is located inside of the elbow and is known as your “funny bone.” It can cause pain, numbness, and tingling, especially to the ring and pinky fingers. 

Carpal Tunnel Syndrome (CTS)

Carpal tunnel is a condition you may have heard thrown around in conversation, maybe from someone who uses their hands and wrists a lot for their jobs. Carpal tunnel syndrome impacts approximately 5% of all Americans, according to the American Physical Therapy Association.

The main job of the carpal tunnel is to protect the median nerve on the inside of your wrist, in addition to the tendons you use to bend the fingers. However, this tunnel can become constricted, and when it does, nerve compression can result, leading to a variety of symptoms, such as numbness and tingling to the thumb, 1st, and 2nd fingers. 

Besides pain to the wrist and hand,  swelling to the hand and fingers is even possible. Carpal Tunnel Syndrome (CTS) can be caused by an injury or by repetitive job-related use, such as assembly-line work, keyboard-heavy work, or jobs that require someone to operate heavy machinery.

Golfer’s Elbow/Tennis Elbow

Tennis elbow refers to pain on the lateral or outside aspect of the elbow, whereas golfer’s elbow refers to pain on the medial or inside aspect of the elbow. 

Both of these injuries are a result of repetitive use injuries and can be attributed to a variety of different tasks, not just the sports they are named after. 

Seeing a physical therapist for these conditions is recommended, because therapy restores the underlying joint and soft tissue limitations that are causing strain to the tendons, and reduces the inflammation quite rapidly.

Elbow Bursitis

At the end of the elbow is a fluid-filled sac called the “bursa” that serves as a cushion to the elbow bone, or “ulna.” 

When that bursa is compressed due to an injury or repetitive motion, it can lead to pain and swelling. Because of this, it is common to see this condition in athletes and students.

Sprains/Strains

Strains refer to muscle or tendon injuries, whereas sprains refer more to the ligaments, which connect bones. 

Sprains and strains typically occur from rapid overstretching of the ligaments or tendons, such as a fall on an outstretched arm, or catching something heavy. It may take some time for you to realize you’ve even sustained one of these injuries, as the pain typically takes a day or two to show itself.

The most common types of arthritis

Arthritis causes pain and inflammation, and it can affect one or multiple joints at once. Osteoarthritis and rheumatoid arthritis are the two most commonly experienced types:

Rheumatoid arthritis

Researchers have come to believe that a person’s medical history, environment, and hormones could all be contributing factors toward the development of rheumatoid arthritis. 

Rheumatoid arthritis develops as an autoimmune response, meaning that the immune system sees the joints as a threat and decides to attack them. 

Rheumatoid arthritis is also referred to as “inflammatory arthritis,” and is also fairly common. Because it is an autoimmune condition, it is common for it to affect the same joints on different sides of the body.

This condition is typically more prevalent in females than males as well. Those living from arthritis typically report soreness around joints, which is worse after prolonged sitting, standing, or inactivity.

If you have rheumatoid arthritis, you may notice that pain can get worse when you move, for example, bending over at the knees to pick something up may be uncomfortable for you. 

You may notice popping or clicking sounds in the affected joint(s) with movement, and the joint may be sensitive or painful to the touch. Arthritis can also cause pain when you exercise or work, and the pain may go away after you stop doing that activity.

Osteoarthritis

Osteoarthritis is typically easy to diagnose, as it is the most commonly experienced form of arthritis. It occurs when the cartilage of the joint wears down, either due to age or overuse. 

This causes pain in the joint, as the cartilage is no longer acting as the thick cushion that it once was. Without a cushion, the bones grind together, which in turn causes an inflammatory response in the joint.

Osteoarthritis can be caused by a sudden injury to the joint, or it can develop after a previous injury has fully healed.  

Let’s say you played football and received a rough blow to your knee. You probably received treatment, recovered, and returned to the game thinking all was well again with your body. 

Although the injury healed, there is a possibility that the damage that occurred to the cartilage or surrounding muscles has resulted in a decrease of support to the joint. The motion of the joint may also have slightly been altered. Therefore, it is still possible for you to develop osteoarthritis from that injury later in life.

This also applies to those who work jobs that require repetitive motion or cause the overuse of specific body parts. For example, say you are a construction worker. You swing a hammer in repetitive motions as a crucial part of your job, you may develop osteoarthritis in the joints of your elbows or hands.  

Being overweight may also put you at a higher risk for developing osteoarthritis, as it adds additional strain to your knee and hip joints.

This happens when too much pressure is put on your median nerve, located at the base of your palm. Your carpal tunnel is a narrow channel, about the width of your thumb, located on your wrist under the palm. It protects the median nerve, as well as the tendons you use to bend your fingers. When excessive pressure is put on the median nerve, it causes crowding and irritation of the carpal tunnel, making it difficult for it to do its job. This, eventually, is what leads to carpal tunnel syndrome.

What causes carpal tunnel syndrome?

What’s causing my dizziness?

The three main types of headaches include:

Tension headaches

Tension headaches are caused by pressure on the "dura mater," a delicate membrane that surrounds the brain. 


 

The dura mater sits close to the small muscles at the base of the skull, which goes into spasm when the dura mater is stressed, transmitting signals of pain to the brain and triggering your headache. 


 

This is the most common type of headache, which can occur due to repeated movements or poor posture in the neck/head. If you've ever seen someone at work holding their head at their desk, it's possibly due to a tension headache.

Musculoskeletal headaches

Musculoskeletal headaches arise when there is a large amount of strain in the neck. This is usually the product of physical imbalances, deficiencies, or injury, while emotional stress (such as stress) can also be a contributing factor.


 

Traumatic damage to the neck, such as whiplash, is a common cause of musculoskeletal headache. In the case of neck injuries such as whiplash, the muscles and connective tissues in the neck may become strained. 


 

This can lead to agonizing pain in the head, as well as in the face, shoulders, or neck. 


 

Cervicogenic headaches

The majority of headaches are rooted in problems with the neck.

What is plantar fasciitis?


 

When understanding plantar fasciitis, it is important to also understand heel spurs. Plantar fasciitis, also colloquially referred to as “policeman’s heel,” can either develop on its own or as a symptomatic response to heel spurs. Heel spurs are calcium deposits that grow on the bottom of the heel and can cause bone-like protrusions to stick out of the flat underside of the heel. This can result in soreness or pain, which can develop into plantar fasciitis.


 

Plantar fasciitis occurs as an inflammation of the “plantar fascia,” which is a long tissue at the bottom of your feet that connects from the heel bone to the toes. Because of the range of tissue, you may feel pain or soreness in your heel, toes, or the entirety of your foot’s underside. People with plantar fasciitis also report their peak pain occurring in the morning, describing it as a “stabbing pain” that seems to alleviate with mobility as the day progresses. It is also important to contact a doctor if you notice swelling or pain at the bottom of your foot, as it may be a sign of plantar fasciitis, and could be a symptom of heel spurs.


 

Am I at risk for developing plantar fasciitis?


 

Like many physical ailments, plantar fasciitis typically forms from physical exertion. What is a rotator cuff injury?

Plantar Fasciitis

The plantar fascia is a band of connective tissue that runs on the bottom of your foot and helps with strengthening your arch to provide a proper push-off when walking or running.. Plantar fasciitis is inflammation of this connective tissue. With this injury you can feel pain throughout the arch or the heel. 


 

Hip Pain

Pain in the front of the hip is common when running on a different surface or using an improperly fit pair of shoes. 

Hip pain also commonly occurs on the outside of the hip, an area called the greater trochanter (which actually isn’t your hip at all). This can be caused by overworked abductor or hip rotator muscles leading to tightening, tendinopathy, or bursitis.


 

Lower Back Pain

Lower back pain from running is commonly caused by a dysfunctional stride that may result in landing too hard causing an increased ground reaction force up your leg to your back, causing compression of your lower vertebrae. 




 

How can I prevent these injuries?


 

While it is always a wise decision to consult with a physiotherapist, there are some tips you can implement into your personal life as well, in order to decrease your risk of sustaining an injury. These include:



 


  • Active Stretching. This is one of the easiest ways to prevent injuries. Your tissues require water in order to be elastic, and proper stretching helps to bring water into the tissues. This increases joint mobility and muscle elasticity and reverses stiffness.


  • Hydrating and eating nutritious meals. Did you know that your body is over 70% water? Because of this, it requires water in order to remain supple and function properly. Without adequate water and nutrition, your body’s tissues are more prone to over-stretching and injury. Make sure you drink the recommended 8 glasses of water a day, and a few extra on hot & humid days. 


  • Staying within your abilities. It is important to train and practice for any physical activity you’d like to try. Start small in the beginning and build up from there. For example, if you aren’t used to running but you’d like to complete a 5k, don’t start off with 3-mile runs. Begin with a half mile and slowly build up. When you push your body past its current limits, you make yourself more prone to injuries because your body is not prepared for the physical demands being asked of it. Understanding strain vs. sprain

Strains and sprains are soft tissue injuries that can lead to debilitating pain. A strain occurs in muscle or tendon tissue whereas a sprain occurs in ligamentous tissue. Tendinitis

What is tendinitis?


 

Tendinitis is known as an “overuse injury,” because it develops over time. Every bone in the body is connected with muscular fibers called tendons. The tendons are flexible, allowing the body to move freely by letting bones stretch apart or move in one direction or another. Tendinitis occurs when the tendons become inflamed, which can result in swelling and pain, especially with movement of the affected area.


 

Typically, when pain is caused as a result of tendinitis, the pain is isolated at the noted areas of the body. For example, a tennis player may experience tendinitis in the elbow or shoulder, whereas a runner may be more likely to experience it in the Achilles tendon. In fact, this is why tendinitis in the elbow is frequently referred to as “tennis elbow,” while Achilles tendinitis is frequently referred to as “runner’s ankles” or “runner’s heels.”

When tendinitis first develops, a good rule of thumb is to practice the R.I.C.E method – rest, ice, compression, elevation. Tennis Elbow

Tennis elbow is a colloquial term referring to an overuse injury that results in inflammation of the tendons in the forearm, making it difficult to lift and move the arm. While it is commonly caused by the sport of tennis, it can also be caused by other sports and activities, including softball, baseball, raking leaves, and even excessive typing on a computer. The name implies specificity to the elbow; however, pain may be felt throughout the entire upper extremity.

How do I know if I have tennis elbow?


 

Patients who experience tennis elbow typically report pain around the bony part of the extremity, just above the outside of the elbow. This is called the “lateral epicondyle.” This area may be tender to touch, and you may experience weakness and/or an inability to perform normal activities of daily living, such as lifting a cup of coffee or reaching for something on a shelf.


 

Any activity, such as gripping or repeated grasping, can strain the tendon attachment to the lateral epicondyle. The most common age group to develop this condition is between 30-60 years of age, although it can occur in younger and older patients.

Workstation/Sitting Ergonomics


 

“Ergonomics” entails the study of people’s efficiency in their working environment.  It can be defined as the way in which we improve movement and function within daily life. Physical therapy with regards to ergonomics focuses on postural awareness and body mechanic strategies that focus on maximizing efficiency, comfort, and safety, in addition to identifying & altering factors that may lead to strain or injury. 


 

There is no such thing as a ‘perfect posture’, our bodies are made to move, not to remain still. Posture is not just a position, but a dynamic pattern of reflexes, habits, and adaptive responses to anything that resists you being more or less upright and functional, such as: gravity, awkward working conditions, and athletic challenges. It stands to reason that any habitual positioning which causes unnecessary strain on the body is generally not great for us. Emotion plays a huge part too - if we are stressed out while we are in that hunched posture at work we are going to feel more pain than if we are hunched over hanging out watching our favorite TV show. 


 

So what can we do about it? Some easy tips to focus on for postural health, instead of just trying to "sit up straight"

1. Moving is way more important than finding that perfect position

Our bodies are made to move, not stay stationary. You should take a break from being in the same position every 15 min. Stand up, walk to the water fountain, use the restroom, or simply shift your weight so you are situated differently than you just were. "The next posture is the best posture"

2. Take your shoes off or choose sensible footwear

Standing desks are pretty cool as they let us follow rule #1 - switch between positions while working. But what sense does it make to stand up wearing insensible shoes that put strain on the feet and the spine? If you can be barefoot at work without getting looked at like you're crazy, go for it; your feet will thank you in the long run. If not, make sure your shoes fit well, are tied nice and snugly, and aren't too old or worn out. Typically shoes last 1.5-2 years before the material starts to wear (even less time if you walk or run in them).

3. The best chair is two chairs and a wobble cushion

This one is a bit on the unconventional side, but consider this: Sitting in a super comfortable chair will encourage more sitting, which we don't want. But, being uncomfortable for too long will cause our work to suffer. Seeing as we want to change positions as much as possible, having one chair that’s more comfortable paired with, say, a stool that isn't as comfortable gives us variability that mixes comfort and attentiveness. Alternating between being more upright on a stool and more laid back in a comfy chair is a great way to spend the day, should you be sitting for most of it.

Add in a wobble cushion (which challenges you to sit more upright otherwise you'll fall over) gives you 4 different sitting positions to rotate throughout the day.

Awareness Through Movement

"Through awareness we can learn to move with astonishing lightness and freedom at almost any age and thereby improve our living circumstances, not only physically, but emotionally, intellectually and spiritually. -" Moshe Feldenkrais


 

Every week is a different lesson. Clothing should be non-restricting and comfortable. Movements may be done lying on mats on the floor.


 

(ATM) sessions are taught in a one on one or group setting. Participants are verbally led through various sophisticated, yet simple, sequences of movements, which are done very slowly and within a comfortable range of motion for each individual. There are over a thousand different movement lessons.


 

Through ATM, individuals learn to recognize detrimental habitual patterns of movement that may be contributing to orthopedic or neurological problems, or inhibiting the ability to perform daily activities. The repetition of these non-strenuous sequences of movements teaches the brain to replace dysfunctional movement and thought patterns with new, more efficient movement options.


 

As one learns to relax and abandon habitual patterns, increased awareness, flexibility and coordination evolve naturally. Participants typically report feeling more relaxed, energized, taller, lighter, and more balanced in posture. Pain and tension often decrease and breathing becomes easier.

Corrective Stretching

Stretching is an effective component of exercise- but only when done properly! A few simple coordinated physical motions can dramatically improve your overall health, wellness, and quality of life.

Passive vs Active stretching

Passive stretching, which is statically holding a position for a given length of time, will take quite a while to create a change, and also cause the shortest-lived effects on muscle length. Research says we would need to hold a position for >5 min (which nobody does) and the small effects would last less than 24 hours. What we need to realize is that muscle length is determined by the nervous system's signals. When something feels like it is going further than the body has control of, signals get sent from the brain to tell it to tighten up, so that the body does not sustain an injury. The only way to improve this is to use mobility drills where you control your body into the end ranges of motion, to change the nervous system's response to being in those "dangerous" end ranges. Change happens in the nervous system with active and purposeful movements, not pulling a muscle apart and hoping it will stay there.

What will a fall prevention PT plan look like?


 

Your initial physical examination at Central Park Physical Therapy may consist of several components to help decide what your weaknesses are.


 

Vision tests, thought tests, resting heart rate checks, active heart rate checks, and assessments of your gait, balance, range of motion, and strength can all be performed.


 

Based on the results of this evaluation, our physical therapist will design a treatment plan for your specific needs. 


 

These plans are aimed first and foremost at reducing your risk of falling, but they will also aid you in improving balance, strength, flexibility, endurance, and overall movement. 


 

Some common forms of treatment include:



 


  • Strength training. Strength training is usually combined with balance training. Our physical therapist will develop a strength training program for you that will concentrate on particular muscle groups that need to be improved. This will boost your standing and walking balance, as well as your ability to recover from a loss of balance.


  • Endurance training. Endurance preparation consists of eventually advancing to more advanced stages of the same type of treatment. Our physical therapist will develop an aerobic exercise routine for you and progressively increase the length of those workouts as your stamina improves. For instance, your endurance training may begin with 10-minute sessions and progress to 30-minute sessions.



 


  • Pain management. If you are experiencing pain in any area, it will be treated as soon as possible in your care plan. Our physical therapist will try to make the recovery as easy as possible, so you will first focus on relieving pain before moving on to any other types of physical activity that may cause you discomfort.



 


  • Balance training. Balance is a large part of fall prevention, as lack of stability is one of the main reasons why falls occur. Our physical therapists will design a balance-training plan for you as part of your treatment and may ask you to perform certain balance-based activities, such as standing on one leg or holding your balance while performing a mentally-stimulating task (such as reciting the alphabet or reading a page from a book.) 



 


  • Walking and moving programs. This part of your treatment plan is aimed at getting you back to your normal physical function when walking and/or moving. Our physical therapist may ask you to perform certain activities, such as walking in a circle or completing an obstacle course.


 

How does physical therapy help decrease fall risk? 


 

A recently published systematic review by Cochrane, comprising more than 100 randomized controlled trials, supports exercise interventions as an effective method of treatment for patients at increased risk of falling. The average age of the patients in this review was 76, and 77% of the patients were female.


 

The risk of fall-related fractures decreased by 27 percent and the number of falls that required medical attention decreased by 39 percent. Fall risk was also reduced by 21-24 percent in this group, depending on whether individual or group treatments had been performed.  


 

Results concluded that those who participated in exercise interventions had a 23 percent drop in fall risk compared to the control group.


 

Concluding statements by the authors showed that overall, "Exercise reduces both the rate of falls... and the number of people experiencing falls."

History of The Feldenkrais Method


 

The Feldenkrais Method was developed through 40 years of research by Moshe Feldenkrais, Ph.D., D.Sc., a Russian-born physicist, mechanical engineer, educator and martial artist. A severe injury led Dr. Feldenkrais to a lifelong investigation into ways of improving the body's ability to function. His innovative method addresses a broad spectrum of muscular, orthopedic, and neurological problems. Awareness Through Movement

(ATM) sessions are taught one on one or in a group setting. Participants are verbally led through various sophisticated, yet simple, sequences of movements, which are done very slowly and within a comfortable range of motion for each individual. There are over a thousand different movement lessons.


 

Through ATM, individuals learn to recognize detrimental habitual patterns of movement that may be contributing to orthopedic or neurological problems, or inhibiting the ability to perform daily activities. The repetition of these non-strenuous sequences of movements teaches the brain to replace dysfunctional movement and thought patterns with new, more efficient movement options.


 

As one learns to relax and abandon habitual patterns, increased awareness, flexibility and coordination evolve naturally. Participants typically report feeling more relaxed, energized, taller, lighter, and more balanced in posture. Pain and tension often decrease and breathing becomes easier.


 

Functional Integration

(FI) involves one-on-one treatment by a Feldenkrais practitioner who moves the patient's head, spine and limbs with his hands through slow, gentle movements specifically tailored to the particular needs of the individual. This extra blood flow helps facilitate the healing properties found in your bloodstream. Cold compresses and packs come to the rescue with their ability to halt spasms, numb pain, and reduce swelling in the affected areas. Some people prefer one temperature over the other.

Ice Packs/Ice Massage

Decreases pain and decreases inflammation.  Helps to circulate or bring “new blood flow” to the affected area as tissues re-warm. This extra blood flow helps facilitate the healing properties found in your bloodstream. Cold compresses and packs come to the rescue with their ability to halt spasms, numb pain, and reduce swelling in the affected areas. Some people prefer one temperature over the other.

Muscle Energy Technique

What is Muscle Energy Technique?    


 

Muscle Energy Technique (MET) improves musculoskeletal function through mobilizing joints and reducing tension in tight muscles and fascia, to reduce pain, and to improve circulation and lymphatic flow. These methods are  active techniques, requiring the patient to perform isometric contractions against resistance.


 

MET uses muscle effort to mobilise joints and tissues, and to facilitate relaxation, restore alignment, and promote pain relief. Muscle hypertonicity is reduced via post-isometric relaxation – the reduction in tone of an agonist muscle following isometric contraction, and reciprocal inhibition – the reduction in tone of an antagonist muscle after its agonist has undergone isometric contraction.

How is it done?


 

Muscle energy technique involves careful positioning of an area of the body by the therapist. After positioning, you will be asked to contract the muscle by pushing into the therapist's resistance in a specific direction followed by relaxation and passive movement into newly gained range.

 

During the treatment session you will be asked to actively participate in facilitating and relaxing muscle groups. Under the direction of the therapist you will learn to lengthen shortened muscle, relieve muscle spasm and reduce tone in spastic muscles.


 

 

 

Muscle energy techniques can be applied safely to almost any joint in the body. METs are also used as a preventative measure to guard against future injury of muscles and joints. Neuromuscular re-education is a technique used by physical therapists to restore normal body movement patterns especially after improvement of range of motion in any joint.


 

Your nerves send signals from your brain to your muscles to produce movement. Over time, muscle movement patterns are learned and stored in your memory.


 

When nerves or muscles experience damage or injury resulting from trauma and various medical and neurological conditions, muscle movement patterns can be negatively affected. Neuromuscular re-education is one method used by physical therapists to facilitate the return of normal movement in patients with neuromuscular impairments.


 

To eliminate many of the symptoms that a patient presents with during their treatment time, it is critical to re-establish normal kinesthetic sense and proprioception. A joint needs to have full motion to be fully functional. If the joint does have full range of motion, weakness and functional limitations will develop over time. This time period is different for every patient, normally several weeks to months when the symptoms are not treated properly. Any loss of motion and strength needs to be addressed to eliminate the patient’s symptoms and correct the underlying problem. 


 

If you were to restore full motion without reeducating the muscle in the new range, the new range would be lost or be weak and susceptible to reinjury. Re-education of the muscle in the new range of motion is necessary to maintain optimal strength and motion.

Why are my feet hurting?


 

Foot problems can be caused by several disorders that are directly connected to the feet. Ailments and diseases that affect the whole body can also cause foot pain.


 

The following are several examples of conditions or diseases that can be the cause of foot pain.  These conditions can all be treated with physical therapy:

Diabetes

Diabetes can lead to foot problems because of restricted blood flow and nerve damage. When nerves are damaged through diabetes this is referred to as neuropathy, which can be particularly painful to deal with.


 

When the muscles in the foot no longer function together properly this can cause unequal pressure on the foot. 


 

Unequal pressure can result in sores, infections, and even amputation in extreme cases. Proper footwear can help prevent foot problems related to diabetes. 



 

Heel Spurs

Many of the same risk factors that cause plantar fasciitis can also cause heel spurs. This is a bony protrusion on the heel that is usually made of some sort of calcium build-up.

Plantar Fasciitis


 

This condition can be caused by obesity, age, and standing for long periods on your feet.


 

It manifests as pain felt along the sole, especially in the region connecting the heel bone to the toes. 


 

This is caused by tightened or inflamed tissues around the bottom of the feet. Pain in the heel or arch of the foot is also possible.


 

Arthritis

Arthritis is one of the most common diseases.  It is a debilitating joint disease that can affect the joints throughout the foot. Stiffness and swelling in the joints can eventually lead to permanent damage. This can affect the way you stand or walk.


 

Metatarsalgia

This disorder is often caused by ill-fitting or restrictive footwear. This is a debilitating disease that affects the joints, bones, and the ball of the foot. 


 

Orthotics are typically used to help alleviate the pain caused by metatarsalgia.


 

Other potentially treatable conditions include patellofemoral knee pain, shin splints, Achilles tendonitis, and bunions, as well as numerous systemic pathologies that (like diabetes) affect the function of the lower limbs.

Soft tissue mobilization (STM) is a form of manual physical therapy where your licensed PT uses hands-on techniques on your muscles, tendons, ligaments, nerves, and fascia in order to release adhesions and optimize your muscle function.

Your muscles are made up of many small, thin fibers that contract on command, working together to mobilize your muscle groups. These muscles are attached to your bones by connective tissues, called tendons, enabling them to move your body parts. Your bones are then connected by tough bands of connective tissue, called ligaments, for added stability. Your ligaments are covered by yet another connective tissue called fascia. All of these connective tissues can be referred to as “soft tissues.”

The importance of soft tissues

Your muscles are made up of many small, thin fibers that contract on command, working together to mobilize your muscle groups. 

These muscles are attached to your bones by connective tissues, called tendons, enabling them to move your body parts. 

Your bones are then connected by tough bands of connective tissue, called ligaments, for added stability. Your ligaments are covered by yet another connective tissue called fascia. All of these connective tissues can be referred to as “soft tissues.”

These soft tissues can be damaged by certain stresses, especially those caused by the repeated actions within the athletic activity. 

A tear in the tendon causes you to experience a significant amount of swelling and inflammation. Many athletes also experience sprains, which are caused by a ligament stretching farther than intended. 

These injuries may occur in response to the intensity of your sport, but they can also occur if muscles simply are too tight before you begin one of your workouts. 

Therapeutic massage can help with this, as it relaxes the muscles, and may even prevent some of these injuries from occurring in the first place!

About Feldenkrais Method

The Feldenkrais Method was developed through 40 years of research by Moshe Feldenkrais, Ph.D., D.Sc., a Russian-born physicist, mechanical engineer, educator and martial artist. A severe injury led Dr. Feldenkrais to a lifelong investigation into ways of improving the body's ability to function. His innovative method addresses a broad spectrum of muscular, orthopedic, and neurological problems. Through simple, gentle movements, one can reeducate the neuromuscular system and learn new, more efficient ways of functioning at work and at play.

ATM participants typically report feeling more relaxed, energized, taller, lighter and more balanced in posture. Daily activities become more enjoyable.


 

The Feldenkrais Method

Chronic pain or recurring injuries are often caused or aggravated by bad habits in our posture, gait, the way we work or play sports, and everyday routines. Awareness Through Movement

(ATM) sessions are taught in a group or class setting. Participants are verbally led through various sophisticated, yet simple, sequences of movements, which are done very slowly and within a comfortable range of motion for each individual. There are over a thousand different movement lessons.

Through ATM, individuals learn to recognize detrimental habitual patterns of movement that may be contributing to orthopedic or neurological problems, or inhibiting the ability to perform daily activities. The repetition of these non-strenuous sequences of movements teaches the brain to replace dysfunctional movement and thought patterns with new, more efficient movement options.
As one learns to relax and abandon habitual patterns, increased awareness, flexibility and coordination evolve naturally. Participants typically report feeling more relaxed, energized, taller, lighter, and more balanced in posture. Pain and tension often decrease and breathing becomes easier.

Functional Integration

(FI) involves one-on-one treatment by a Feldenkrais practitioner who moves the patient's head, spine and limbs with his hands through slow, gentle movements specifically tailored to the particular needs of the individual. The Feldenkrais practitioner can detect restriction, tension and rigidity and through this series of very calculated, minute movements, facilitate freedom of movement in the joints and muscles and reduction of pain. Why do I have hip joint pain?

Your hip is a ball-and-socket joint that works to support the weight of your upper body, relying on multiple muscles and tissues to keep it mobile and stable so it can function properly. 

Pain felt in the hips may originate in the joints themselves, but it may also be a result of an underlying condition in another part of the body. For example, your hips are part of a kinetic chain, meaning they make up a combination of weight-bearing joints that must function with other parts of the body in harmony (such as the knees), in order for your body and posture to function properly. 

This means that while you may be feeling pain in your hip, the problem could actually be rooted in a different part of your body. Therefore, a problem with your knee joint may transmit painful signals to your hips, and vice versa. If one part of the kinetic chain is out of balance, stress and deterioration may be placed on another.

What causes hip pain?

Overuse injuries, such as tendinitis and chronic muscle strain, are common in the hips because they are constantly in use. However, the hips may also be subject to acute injuries, such as sprains, strains, and dislocation.

Hip pain can also be affected by referred pain from a pinched sciatic nerve, as the nerve travels through the area. Imbalances in your stance or gait may cause abnormal stresses and premature wear-and-tear in your hips, resulting in painful symptoms or arthritis.

The Mayo Clinic states that cartilage injuries, known as labral tears, are specific to hip pain. However, painful injuries that cause instability in the hips can also affect parts of the body that are located in the same kinetic chain. For example, tight hip flexor muscles and weak gluteus medius muscles can cause the hip to rotate inward without you realizing it. This can cause painful problems such as iliotibial band friction syndrome.

Additional causes of hip joint pain may include fractures, bursitis, certain cancers, or avascular necrosis (also known as osteonecrosis). Avascular necrosis is a condition that occurs when blood flow to the hip bones slows down, causing the bone tissues to die. This condition happens most commonly in the hips and can develop as a result of a dislocation, fracture, or long-term use of high-dose steroids.

How will Feldenkrais Method relieve my hip pain?
